Application Form
A form is to be filled comprising of the pupil’s full name, date of birth, code number (especially for those joining classes between standard 2 and standard 7), medical conditions (if any), and previous school name with a reason for leaving.
Finally, the parent or guardian is to indicate their name, contact details, and residential address. They must sign as an agreement and commitment to take full responsibility for the child’s fees and all school-related activities.
